The John J. Pershing VA Medical Center, Poplar Bluff, MO is currently accepting applications for current and future Registered Nurse vacancies. Multiple appointment types and locations are available. John J. Pershing VA Medical Center is a Level-3 complexity medical center with six (6) Community Based Outpatient Clinics. This vacancy announcement will be used to create an applicant pool. Qualified applicants will be referred as vacancies occur. More than one vacancy may exist at a location. PACT RNs practice as an RN Care Manager in the Primary Care Clinics. The RN Care Manager is a member of the Patient Aligned Care Teamlet (PACT). S/he uses the nursing process and evidence-based practice to collaborate with the Teamlet (Patient, Primary Care Provider, RN Care Manager, Clinical Associate and Clerical Associate) and expanded Team (Family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven care plan. The RN Care Manager is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patient's needs. The RN Care Manager provides patient and family health education with a focus on self-management, prevention and wellness based on the patient's goals. Community Living Center (CLC) RNs provide care in long term care, short-stay continuing care, short-stay rehabilitation, short-stay skilled nursing, respite and end of life care. Acute Care RNs are responsible for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements and evaluates care based on age-specific components. Responsible for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge and supporting factors that impact customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values and previous experiences. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Acute Care RNs provide care in an in-patient setting. Work Schedule: Multiple work schedules are available and is dependent upon the area/location of the position.
