Relocation/Recruitment Incentives are NOT authorized. Home Sweet Home Neighborhood is staffed 24 hours a day to provide medical care for long term care residents. The nursing staff includes RNs, licensed practical nurses and nursing assistants. The population served includes Veterans with a variety of medical and psychiatric diagnoses. Veterans range in age from adult to frail elderly. The length of stay is considered long-term or indefinite. The CLC RN is responsible and accountable for all elements of the nursing process when providing and/or supervising direct Veteran care. Assesses, plans, implements and evaluates care based on age-specific components. The CLC RN assumes responsibility for the coordination of care focused on Veteran/resident transition through the continuum of care, Veteran/resident and family education, and Veteran/resident self-management after discharge, and supporting factors that impact customer satisfaction. The CLC RN considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. The CLC RN administers medications including intravenous therapy (IV insertion and IV infusions), carries out other procedures per established policies and guidelines. The CLC RN participates in discharge rounding and provides clinically relevant information to the clinical team. On a daily basis all RNs in this position are expected to perform the usual and customary duties of a front-line nurse including but not limited to physical assessment, wound and skin assessment/care, IV therapy, medication administration, and general nursing management of the resident's physical and emotional well-being. The nursing process of assessment, planning, implementation and evaluation is applied on an ongoing, systematic manner to all Veterans/residents. RNs are actively involved in providing, developing and facilitating Veteran/resident education. The neighborhood and nursing staff also support other areas within the medical center as needed and accompany residents upon transfer to other facilities if necessary.
