The function of the Registered Nurse is to provide, direct and supervise the delivery of quality nursing care to a culturally diverse veteran population. The Registered Nurse possesses a broad-based clinical knowledge and skills to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting. The Registered Nurse effectively implements activities towards health promotion and prevention, management of acute and chronic illness states, assisting the veterans in attainment of optimal levels of functioning through rehabilitation. The Home-Based Primary Care RN and Spinal Cord Home Care RN provide for care planning and assessment, care coordination and direct patient care in veterans' or caregivers' private residences such as in the Medical Foster Home Program and assisted living facilities in collaboration with the HBPC multidisciplinary team. The HBPC/SCI-HC RN's role includes initial and ongoing assessment/reassessment, monitoring and care coordination during the veteran's entire enrollment period to meet the patient's healthcare needs utilizing the principles of case management. The HBPC/ SCI-HC RN ensure smooth transition of care to and from home care through diligent and collaborative efforts with multidisciplinary teams within and outside the facility. This would also include co-management of care with outside community agencies, particularly with dual care providers. She/he communicates and coordinates care within the facility and community healthcare agencies including state and federal regulatory agencies as it relates to maintenance of safety and compliance with nursing standards of care and practice. The program provides for longitudinal care whereby the veteran may stay in the program for the duration of his lifetime.
