Duke Homecare & Hospice Duke Homecare & Hospice offers hospice, home health and infusion services as well as serves as the home for the Duke Caregiver Support Program.
Our Hospice team works closely with a patient's physician to provide comprehensive, individualized care in the comfort their home or at our inpatient hospice facility located in Durham.
Our team of nurses, therapists, social workers, counselors, and chaplains help patients and their families manage advanced medical conditions.
Duke Home Health provides services in a patient's home that include skilled nursing, physical therapy, occupational therapy, speech therapy, social service, home health aides, and pain and wound management.
Our infusion team comes to a patient's home to administer intravenous medications, including antibiotics and chemotherapy, to children and adults. Our nurses, pharmacists, dietitians, and patient services coordinators work under the direction of a patient's doctor to monitor needs and treatment.
JOB SUMMARY
In support of a department or group of professionals, clinical staff and patients, regularly required to resolve moderately complex (non-routine) administrative problems independently and with others. Problem solving may include research and tasks for this role are typically more diverse and advanced in nature.
MAJOR JOB RESPONSIBILITIES
Implement appropriate information systems and work processes in support of referral center activities.
Independently prepare more complex documentation, reports, charts, graphs and spreadsheets at designate intervals and as requested.
Process new referrals including building of chars in Netsmart and Wellsky
Provide administrative support for the care coordination of referrals
Prioritize, triage and resolve inquires (phone, email, in-person)
Create and manage charts for multiple DUHS entities (Duke Hospice, Home Health & Infusion, via Netsmart and Wellsky)
Manage DUHS and Physician/Community referrals, UNC
Communicate emails/documents received to appropriate teams
Monitor referral center intake IT system and triage referrals to appropriate personnel and departments.
Input patient referral information into referral center intake IT system as needed.
Input patient referral information into appropriate clinical IT programs.
Input referral source information into appropriate database, (excel). Maintain referral source database for all three clinical programs and as DHCH as a whole.
Monitors and updates physician licensure and NPI data as required.
Assist in triage of phone referrals as appropriate. (not evaluate and take complete referral.
Copy and distribute referral information for clinical programs.
Plan and schedule work priorities in accordance with program goals and objectives and deadlines.
Maintains confidentiality of employee and patient information.
Exhibits behaviors that promote DHCH's co-worker service standards.
Displays a clean, neat, professional appearance.
Participates in Performance Improvement Program.
Perform other related duties incidental to the work described herein.
Performs other duties as assigned.
Minimum Qualifications
Education
High school education or equivalent.
Experience
Two years of administrative support experience of increasing variety and/or complexity
